Análise das pressões intracuff em diferentes angulações da cabeceira do leito dos pacientes na terapia intensiva

Abstract

Introduction: The use of the ventilation invasive mechanics (VIM) requires use of endotracheal tube (TOT) and/or tracheostomy. These have a ballonet with the function to forbid the trachea, to keep adequate the pulmonary ventilation and to prevent the gastric aspiration. Methods: A transversal study was made, analyzing and comparing the different pressures intracuff in angulation of zero, 30 and 60 degrees of inclination of the bed rest of the patients in UTI using TOT or tracheostomy in VIM. The pressure intracuff, the current volume exhaled (Vt) and the pressure of peak of the airways (PP) in the cited positions had been analyzed, of which they had been chosen of random form, with exception of the position of 30º that it was considered of reference (control). Results: The sample was composed for 12 women and 12 men, with average of 51,29 age of ± 19,55 years. When modifying the inclination of the bed rest of 30º for 0º, had an reduction of 16,9% in the pressure intracuff and an increase of 11,8% in the PP. Already in the alteration of 30º for 60º, the pressure intracuff reduced 18,8% and the PP had increase of 13,29%. These findings had been significant (p<0.05), however did not have significant alteration in the Vt with the position changes Conclusion: It is necessary monitorization and adequate adjustments of the pressure intracuff, in all moments where the patient will be submitted to the inclination of the bed rest, to make possible the prevention of the air escape and the occurrence of nosocomial pneumony
